BLACK-AND-WHITE OR COLOR

Pages 4 to 19 are black-and-white sheets, low ink, made for unit printers; each setting has a small tab that prints as a different gray. Pages 20 to 23 repeat four sheets with a full color band and tinted headers for color printers (they use more ink). The color pages are in all four PDFs, so the file names below stay the same.

HOW THE FILLABLE COPY WORKS

Only the unit set-up, the timeline hours and the med pass times are type-in boxes. Everything about a patient is handwritten on the printed sheet, by design. Timeline hours are pre-filled and editable, so a 0600 or 1100 shift works. Shift length is a dropdown (8h, 10h, 12h) that also accepts your own short entry.

PRINTING

Black on white with strong rules. Print at 100 percent (Actual size). The fillable copy opens best in the free Adobe Acrobat Reader, Chrome, or Edge.
